Requirements for Application for Entrance Examination

For In Coming Freshmen

          An Applicant must comply with the following requirements:

A.   Must be a graduate or a graduating student of any secondary school (P.E.P.T passers included)

B.   Must have passed all the subjects, preferably with an average grade of 80%

C.   Must not have any derogatory record from secondary school he/she attended

D.   Must have submitted the following documents/requirements:

1. Certified photocopy of Form 138 (High School Card) containing at least for the second grading second period (For the graduating high school students)

2. Certification of Residency fro the Barangay Captain

3. Certification of Good Moral Character from the Principal /Guidance Counselor/Adviser of the last secondary school attended

          4.  Recommendation Letter from the Principal or from the Fourth Year Adviser

          5. Two (2) copies of 1x1 pictures

For Transferees

            Student – applicants from other colleges/universities will be allowed to apply upon submission of the following:

A.   Recommendation Letter from Dean of the college/University where the applicant is previously studying

B.   Certification of Good Moral Character from the Guidance Counselor of the College/University last attended

C.   Two (2) copies of 1x1 pictures

 

ADMISSION REQUIREMENTS

For In Coming Freshmen

A.   Passing the Entrance Examination and the interview given by the Committee on Admission and Testing of this College

B.   Submission of the original copy of the High School Card

C.   Submission of photocopy of the birth certificate duly authenticated by the Local Civil Registrar of the place where the applicant was born

D.   If married, photocopy of marriage certificate

E.   One long brown envelope

F.   One long size mailing envelope with stamp

For Transferees

A.   Passing the Entrance Examination and the interview given by the Committee on Admission and Testing of this College

B.   Submission of the Honorable Dismissal and Transcript of Record

C.   Submission of photocopy of the birth certificate duly authenticated by the Local Civil Registrar of the place where the applicant was born

D.   If married, photocopy of marriage certificate

E.   One long brown envelope

F.   One long size mailing envelope with stamp
